THE ROLE OF LAPAROSCOPIC SPLENECTOMY IN SURGICAL HEMATOLOGY

The investigation in the clinic was experimentally performed which proved unquestionable superiority of laparoscopic splenectomy (L.S.) in comparison with traditional laparotomy spleen removal in hematological patients. One of the main aims of the research was laparoscopic splenectomy safety increase. Clinical part of the experiment was preceded by experimental research on dogs, aimed to study influence of splenic artery ligation on the state of the spleen itself. We also studied topography of the spleen and pancreas blood supply in corpses.
12 laparoscopic splenectomies were performed during the period from 2006 till 2007 in the clinic. All the patients were referred to the planned operative treatment after thorough examination and determination of all indications for the operation at the hematological department. In 10 patients idiopathic thrombocytopenic purpura was revealed; in 1 – anemia, in 1 – microspherocytosis. Preliminary endovascular occlusion of the splenic artery and the spleen surface treatment with surgical laser were applied to increase operation safety. In the stage of spleen mobilization and vessels ligation in porta lienis US scalpel and LigaSure apparatus were used. There were no any complications connected with embolisation. Intra-operative hemorrhage of the splenic vessels and tissues wasn’t observed. Average duration of the operation was 145±4,4 minutes. Duration of post operative therapy period at the in-patients department comprised 5±0,14 days. There were no lethal outcomes.
