СТРАТЕГИЯ И ТАКТИКА АHТИБАКТЕРИАЛЬHОЙ ТЕРАПИИ ПРИ ОСТРОМ ДЕСТРУКТИВHОМ ПАHКРЕАТИТЕ

Актуальность обсуждаемой проблемы определяют прогрессивно увеличивающееся число больных острым деструктивным панкреатитом, высокая частота развития разнообразных внутрибрюшинных и экстраабдоминальных гнойно-септических осложнений, закономерно определяющих высокий процент неблагоприятных исходов. К настоящему времени общеизвестен тот факт, что инфицирование очагов деструкции в ПЖ и ППК происходит у 40–70% больных ОДП в различные сроки заболевания. В обзоре представлено обсуждение круга вопросов, связанных с проблемой адекватного и рационального применения антибиотиков при ОДП в зависимости от их пенетрирующей способности в ткань поджелудочной железы. Обсуждаемая проблема приобретает всё большую значимость в связи с активизацией хирургической тактики, невозможностью одномоментной санации всех гнойно-некротических очагов, использованием многоэтапных некрсеквестрэктомий и программируемых вмешательств, которые являются дополнительным фактором риска нозокомиального инфицирования при ОДП. При назначении антибиотиков следует учитывать то обстоятельство, что успехи современной интенсивной терапии позволили уменьшить летальность в острую фазу панкреатогенного шока, однако при этом возрастает возможность формирования гнойно-септических постнекротических осложнений в позднем периоде течения ОДП. В обзоре рассматриваются показания для двух тактических подходов к применению антибактериальных препаратов – профилактического и лечебного.
