АКТУАЛЬHЫЕ АСПЕКТЫ ДИАГHОСТИКИ И ЛЕЧЕHИЯ АТЕРОСКЛЕРОЗА АРТЕРИЙ HИЖHИХ КОHЕЧHОСТЕЙ У ПАЦИЕHТОВ С СИHДРОМОМ ДИАБЕТИЧЕСКОЙ СТОПЫ

Атеросклероз артерий нижних конечностей – частая патология при сахарном диабете, способствующая развитию ишемической и нейроишемической форм синдрома диабетической стопы. Ранняя диагностика поражения сосудов способствует снижению числа сердечно-сосудистых осложнений, выполненных ампутаций и повышению качества жизни данных пациентов. В обзоре представлен существующий на сегодняшний день арсенал современного комплексного обследования пациентов с синдромом диабетической стопы, включая клинические и функциональные скрининговые, неинвазивные и инвазивные методы лучевой диагностики, а также специфические оценочные шкалы и новые классификации, позволяющие детализировать и систематизировать трофические изменения стопы с возможностью прогнозирования течения патологического процесса. Использование открытых методов реваскуляризации помогает достичь в большом проценте случаев заживления диабетических язв, но проблемы, связанные с локализацией поражения, выбором кондуита для шунтирования и техническими аспектами, значительно ограничивают возможности этого метода лечения. Технический прогресс в развитии эндоваскулярной хирургии расширяет показания для успешной реваскуляризации. В статье освещены результаты открытых шунтирующих и эндоваскулярных методов в лечении атеросклеротического процесса в инфраингвинальном бассейне у пациентов с синдромом диабетической стопы.
