СКОПЛЕHИЯ ПРИ ОСТРОМ ПАHКРЕАТИТЕ

Принятые в 1992 г. на Симпозиуме панкреатологов в Атланте термины, такие как острое жидкостное скопление, панкреатический некроз, псевдокиста, панкреатический абсцесс использовались и используются непоследовательно, без должного соответствия классификации.
Более глубокое понимание патофизиологии некротизирующего панкреатита, улучшенные методы визуализации, накопление результатов исследований сделали возможным рабочей Группе по изучению острого панкреатита в 2012 г., выработать новую классификацию, подразделяя скопления при остром панкреатите на 4 вида: острое перипанкреатическое жидкостное скопление, псевдокиста, острое некротическое скопление и отграниченный некроз. В отличие от положений симпозиума в Атланте новая классификация подразделяет острое некротическое скопление и отграниченный некроз, подчеркивая, что острое некротическое скопление развивается в первые 4 недели острого панкреатита, а отграниченный некроз позднее этого срока. Каждый тип скопления может быть стерильным или инфицированным. Острые перипанкреатические жидкостные скопления возникают на раннем этапе заболевания и большинство их разрешаются спонтанно. Если острое перипанкреатическое жидкостное скопление существует дольше 4 недель, его следует называть псевдокистой. Вмешательства показаны только при доказанном инфицировании скопления, в противном случае показано консервативное лечение. Нарушение протоковой целостности поддерживает существование всех типов скоплений и влияет на исход дренирующих вмешательств. Оперативное вмешательство оптимально выполнять позднее 3-4 недели, когда произошла демаркация некроза. Малоинвазивные вмешательства такие как, лапароскопические, чрескожные ретроперитонеальные и эндоскопические по эффективности равноценны хирургической открытой некроэктомии.
