COMPARATIVE ANALYSIS OF THE CONSERVATIVE AND OPERATIVE TREATMENT RESULTS IN PATIENTS WITH CHRONIC OBLITERATING DISEASES OF THE LOWER LIMBS ARTERIES
Objectives. To study immediate and long-term results of different methods of treating patients with chronic obliterating lower limbs diseases. (COLLD)
Methods. 191 patients with COLLD are included into the research. The subjects were divided into three groups. The conservative treatment only was applied in the 1st group. The operations of the indirect revascularization (lumbar sympathectomy, autohemoextravasation, periarterial sympathectomy, revascularizing osteotrepanation) were carried out in the 2nd group. In the 3rd group the reconstructive surgeries on the arterial bed (aorto-femoral bypass, ilio-femoral bypass, femoral-popliteal bypass surgery, endarterectomy) were performed.
Results. The best results are found out to have been obtained in the group of the patients after the direct revascularization of the lower limbs, the worst results - in a group of conservative treatment. Immediate and distant results after indirect revascularization operations occupy an intermediate position, but when comparing they are closer to the results of treatment in the third group of patients.
Conclusions. Indirect revascularization surgery is effective in treating COLLD and can be recommended for widespread use in the non-surgical wards of city and regional hospitals.
